2 round modules on the headliner right above the B-pillars and 2 straight modules in the hatch/trunk. a 6LED bulb in the from dome light and a 9LED bulb in the back dome light, both those from superbrightleds.com

no idea what happened there, they were just mounted to a flat surface of plastic in the hatch (same place as the straight modules from the first post)

the other cathode looks fine and i never noticed the cathode being dim or half lit, kinda freaky.

they were in the hatch underneath that cover you see in the pics, above and to the sides of the amp, they were in that same spot as the straight LED modules are now.

ive had the cathode in there forever with the other 3 i was using. 2 12" and another 4" in addition to the one that melted.

the other 4" looks perfectly fine and the 12" are perfect as well. and i never noticed anything wrong with it before, they all work fine when i took them off to install the modules. i only noticed the melting when i pulled it off
